Case Manager

Our Case Manager assists with the daily operation of the Nursing Office, oversees medication ordering, care planning, assists with transportation arrangements, and works with physicians and families to assure appropriate care delivery and notifying for medication refills; oversees and maintains compliance with medication management and care delivery; is responsible for maintaining regulatory compliance with regards to the nursing department, for maintaining resident medical files, and for participating in quality assurance activities.

Position Requirements:

  • Minimum of one (1) year experience in home health care, assisted living care, hospital or long term care environment
  • Graduate of an approved college or school of nursing
  • Maintains a current state license as a professional Registered Nurse (RN) (Connecticut Only) or Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
  • Passion for caring for the elderly
  • Demonstrated knowledge of good assessment skills
  • Ability to handle multiple priorities
  • Possess written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care
  • Demonstrates good judgment, problem solving and decision making skills
  • Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications
  • Prior Leadership experience (preferred)
  • Ability to work weekends, evenings and flexible hours
  • Acts as SALSA Designee in the absence of SALSA (Connecticut Only)
  • Shares on-call responsibilities with SALSA  (Connecticut Only)
